The deadline to submit the Westfall Preschool Interest Form has passed. If you have already submitted your form, please see below for further instructions. If you have any questions, please contact our front office at 740-986-4008.
Westfall will begin setting screening appointments and collecting income verification in March 2025. Once the children's screenings are complete and income is verified, eligible families will receive a tuition notice and a full enrollment packet, which must be completed and submitted by May 1, 2025.
Once provided with the full enrollment packet, you will return the completed packet with the following documents to be copied for your child's enrollment file:
Child's Birth Certificate
Child's Social Security Card
Child's Immunization Record
(2) Proofs of Residency
Rental/Lease Agreement or Mortgage Coupon
Utility Bill (only one utility bill may be used) or
Ohio Driver's License with current address
Completed Physical Form
Completed Dental Form
All completed preschool enrollment packets must be returned to the front office by May 1, 2025.
